Isabella County Health Improvement Fund

The Isabella County Health Improvement Fund was created in 2015 by the Board of Central Health Plan. The Fund will provide support for medical health initiatives throughout Isabella County. While this support will be available for all, the main goal of this fund is to support at-risk or low-income residents within Isabella County. Such initiatives will include, but are not limited to, access to care, population health education, population health strategies, and health needs assessments.
